To question 1, the answer is yes and no. The 1966 model year had transitional doors. The earlier '66 cars had the same door handles as a '65, and the late '66 doors had handle that resembled '67s. So the answer is not all '66 doors are equal. However '65 doors should directly replace both styles.
As to removing a rusty hinge screw, you can heat it cherry red and use an impact screwdriver, or simply drill it out.
